Did you know that your baby gets disease immunity (protection) from you during pregnancy? This immunity will protect your baby from some diseases during the first few months of life, but immunity decreases over time. Babies need to be vaccinated starting at birth to stay protected against 14 serious and potentially life threatening diseases. Learn about the caccines your baby needs to stay protected against potentially harmful diseases from the CDC website.

ATTENTION ALL PARENTS:

DON'T WAIT! VACCINATE EARLY!

Prior to entry, attendance or transfer to preschools, schools (K-12), licensed childcare facilities, and family daycare homes, each child shall have on file a Florida Certification of Immunization, DH 680 Form documenting the following:

A. Public/Non-Public Schools K-12:

     -Four or five doses of diphtheria, tetanus, and pertussis vaccine
     -Three, four or five doses of polio vaccine
     -Two doses of measles, mumps, and rubella vaccine
     -Two or three doses of hepatitis B vaccine
     -One dose of varicella vaccine
     -Two doses of varicella vaccine

B. Public/Non-Public Pre-K (age-appropriate doses as indicated):

     -Diphtheria, tetanus, and pertussis vaccine
     -Polio vaccine
     -Measles vaccine
     -Mumps vaccine
     -Rubella vaccine
     -Hepatitis B vaccine
     -Varicella vaccine
     -Haemophilus influenzae type b (Hib) vaccine

C. Public/Non-Public Schools Seventh Grade:

In addition to all other compulsory school immunizations, children
entering, attending, or transferring to the 7th grade in Florida schools
are required to complete the following:

      -One dose of tetanus-diphtheria-pertussis vaccine (Tdap)

D. Licensed Childcare Facilities and Family Daycare Homes:

Age appropriate doses as indicated in accordance with the current
Recommended Childhood Immunization Schedule:

     -Diphtheria, tetanus, and pertussis vaccine
     -Polio vaccine
     -Measles vaccine
     -Mumps vaccine
     -Rubella vaccine
     -Varicella vaccine
     -Haemophilus influenzae type b (Hib) vaccine
     -Pneumococcal Conjugate vaccine

How to: Reminder Recall in FL Shots

 Reminder Recall is one of the many Florida SHOTS™ tools that help you boost immunization levels in your practice. With Florida SHOTS, your office can run reports that show a list of your patients who are due or overdue for immunizations. Reminder Recall allows you to sort and store shot records in a variety of ways (e.g., by type of shot) and print patient information on mailing labels to send reminder notices to parents specifying the shot that is due or overdue.

Advisory Committee on Immunization Practices (ACIP) Recommendations

 

Important Updates:

 

• Effective school year 2011/2012: A fifth dose of poliovirus vaccine is required if the fourth dose was administered prior to the fourth birthday.

• Effective January 2011: The age for the pneumococcal conjugate vaccine requirement has been extended to 59 months as reflected in the recommendations of the Advisory Committee on Immunization Practices (ACIP).           

• Timing of Tdap:

Tdap can be administered, when indicated, regardless of interval since the last tetanus- or diphtheria-toxoid containing  vaccine.

• Adults Aged 65 years and Older:

Those who have or anticipate having close contact with an infant aged less than 12 months should receive a single dose of Tdap.
Other adults ages 65 years and older may be given a single dose of Tdap.

• Children aged 7 through 10 years:

Those not fully vaccinated against pertussis and for whom no contraindication to pertussis vaccine exists should receive a single dose of Tdap.
Those never vaccinated against tetanus, diphtheria, or pertussis or who have unknown vaccination status should receive a series of three vaccinations containing tetanus and diphtheria toxoids. The first of these three doses should be Tdap.

Certificate of Immunization, DH 680

The Certificate of Immunization, DH 680, can now be printed on white or blue paper.  Schools and childcare centers should accept the DH 680 if printed on white paper.
